Grand Prize: (1 winner) 3-day/2-night trip for two to New York City for a spa weekend. Grand Prize trip includes: # Round-trip coach-class air transportation for winner and one (1) guest between the major United States gateway airport closest to winner’s residence and a New York City area airport; provided, however, if winner resides within 250 miles of New York City, air fare shall not be provided to the winner, but instead winner shall be provided ground transportation between winner’s home and the New York City hotel, as selected by Sponsor in its sole discretion and provided further, the type and schedule of the air transportation, as well as the actual airport and airline, shall be determined by Sponsor in its sole and absolute discretion, and the prize winner will be responsible for any fees or additional costs, if any, associated therewith including, but not limited to, up-charges for seat changes, food and beverage, baggage charges, and any flight itinerary changes made after travel plans have been confirmed; Round-trip ground transportation for the winner and one guest between the New York City area airport and the hotel; provided, however, if winner resides within 250 miles of New York City, ground transportation between the airport and the hotel shall not be provided to the winner; One standard room double occupancy for two consecutive nights at a 3-4 star hotel in New York City, selected in Sponsor’s sole discretion; $750 meal allowance, awarded in the form of a VISA or AMEX gift card; and Spa treatments for winner and guest. Winner is solely responsible for all costs and expenses of the prize trip not specified as included in these Official Rules, including but not limited to tips, gratuities, meals, additional ground transportation, souvenirs, local and long-distance telephone calls, hotel room service, laundry service, alcoholic beverages, service charges, taxes, insurance, and other personal expenses. Trip (including, without limitation, air transportation, accommodations, and spa treatments) is subject to blackout dates, availability and additional restrictions. All travel must be completed before December 15, 2011, or prize will be forfeited in its entirety with no compensation or substitution of prize. Winner and guest must travel on same itinerary. Winner and guest are solely responsible for obtaining (and traveling with) valid photo identification and any other required documents. Winner agrees to maintain her behavior in accordance with all applicable laws and generally accepted social practices in connection with participation in any prize activity, including, without limitation, use of a prize. If a winner elects to take trip without a guest, components of the prize will be awarded in increments/quantities suitable for the actual number of travelers and winner will not receive any compensation or substitute/alternate prize for same. Guest must be age 21 or older at time of travel. Gift cards are subject to all terms and conditions generally applicable thereto. Sponsor will not replace lost or stolen gift cards. All federal, state, local, and any other taxes associated with the prize are the sole responsibility of the Grand Prize winner, who will receive an IRS Form 1099 reflecting the actual final retail value of her prize. Prize must be accepted with all listed prize components subject to the conditions indicated herein. No cash in lieu of prize. Prize may not be sold or transferred to any other person or entity. No prize substitution allowed; however, Sponsor reserves the right in its sole discretion to substitute prize (or any prize component) with another prize/prize component of equal or greater value if advertised prize/prize component becomes unavailable for any reason. Actual value of a prize may depend on city of departure, travel dates, changes in value of prize components, inflation, and other factors. Any difference between the estimated value of the prize and the actual value will not be awarded to winner. Once scheduled, trip must be taken on dates and times arranged by Sponsor or prize will be forfeited in its entirety. Changes are not permitted after air ticket issue. Sponsor is not responsible for travel delays or cancellations. There shall be no compensation or prize substitution if winner or guest is unable to take or complete the scheduled trip by reason of illness, family emergency, personal events, terrorism, labor strikes, weather, or other acts beyond Sponsor’s control.
