Sweepstakes Prize: ONE (1) GRAND PRIZE: Winner will receive up to two (2) CoolSculpting® treatments limited to a $1500 value or a $1500 cash alternative (if there is no CoolSculpting treatment center close to winner’s home or if winner is not assessed as a candidate for the treatment).
CoolSculpting treatment will be awarded in the form of a voucher. Winner must complete their CoolSculpting treatment within one (1) year of accepting the prize. The Grand Prize winner will be required to have a free consultation with a CoolSculpting Center (“Provider”) to ensure each Grand Prize Winner is qualified candidates for the procedure. Provider for consultation and free procedure will be chosen at the sole discretion of Sponsor. Grand Prize Winner may have to travel depending on the location of the nearest CoolSculpting Provider. Only a licensed physician with the network of Providers has the ability to determine and prescribe the procedure, and all decisions of that physician are final and binding. Approximate Retail Value (“ARV”): $1,500.
